ERRS Forms
The majority of the forms that you might need to use can be found at 'whostheref.com'. There are however two main ERRS forms that you will also have occasion to use and they are listed below.
Forms
- Self-assessment forms
- It has been agreed to roll out the development programme to all Society referees for the remainder of the season. During the close season we shall review the findings and determine how we progress. A major part of the new approach is a Referee Self Assessment form. This form should be completed by each referee following a game and e-mailed to Bill Mitchell at: bill@ghost-partnership.com. Each form will be reviewed and will be linked to every coaching intervention that takes place. When you are assessing your performance you should do so against the background of the level of game you are doing. The competence framework helps you identify what competence levels you should be displaying relative to your experience.
- Travel Expenses claim form
- Details of how to claim travel expenses can be found on the Expenses page. The necessary forms are linked from that page and are also available from the Downloads pages.
- Referee Abuse report form
- This is the form that you need to fill in if you have been subjected to verbal, physical or other abuse during the course of your refereeing duties. Click here to download this form.
